Volunteer Contact Information - Allows us to communicate with you and to keep you updated on all things Smart Art + Craft Supplies.
Background Check - Any person above the age of 18 years old must pass a background check to be eligible to volunteer at Smart. We have a duty to protect the people with intellectual disabilities that we support and this background check is a very necessary function of doing so.
Photo Release Form - We love taking photos around the store to help us memorialize the work we are doing and promote it. We need your consent to capture and use your photo!
Confidentiality Agreement - Being a volunteer at Smart can mean that you are privy to or even over-hear confidential information about the people we support. We ask for your cooperation in keeping private information to yourself.
Key Holder Agreement - This form is ONLY required if specifically noted by store management. It is not necessary for every volunteer to possess a key to the store. Unless you were specifically asked to complete this form, please skip it.

I give Smart Art + Craft Supplies permission to reproduce and use photos and videos of me on SmartArtAndCraft.org, social media, print marketing and other media release materials.

4. CONFIDENTIALITY AGREEMENT

Progress, Inc. (Smart Art + Craft Supplies)

Consistent with HIPAA and HITECH rules, Progress, Inc. has a legal and ethical responsibility to safeguard the privacy of all person(s) supported by maintaining the confidentiality of all protected identifiable information (relating to their support, care, treatment, condition or payment) which is transmitted, maintained or displayed in any form or medium, whether oral, written or electronic.

In the course of my relationship with Progress, Inc. (Smart Art + Craft Supplies), I may hear, see or obtain Confidential Information during the care, service provision or governance of a client. Such Confidential Information is protected and enforced by federal and state laws in addition to Progress’ policies and procedures. Such information may be accessed, collected, used, communicated or disclosed only in compliance to the minimum amount necessary to perform duties, if any, for which I am responsible.

Confidential Information includes, but is not limited to, the following information of the patient/client:

Name, address, telephone numbers, age, gender, marital status, insurance information, medical information, psychological information, care or service information, or any other identifying information.

Acknowledgement of Confidentiality Agreement

I acknowledge that I have read and understand Progress’s policies regarding confidentiality and agree to comply fully with these policies. Furthermore, I understand that failure to comply with the privacy protections of this agreement may result in disciplinary action including the termination of my employment or other professional agreement. I understand that I may be held liable legally or by any licensing or governing board for a breach of client confidentiality.

I understand that this agreement remains in effect during any and all times that I may have an employee/contractual/professional/student/volunteer/business relationship with Progress, Inc. and that my obligations under this agreement will continue after the termination of my relationship with Progress, Inc. Upon termination, I will immediately return any documents or media containing confidential information if such is in my possession.

5. KEY HOLDER AGREEMENT

Being a key holder at Smart Art + Craft Supplies means that store management trusts you with the responsibility of protecting all property of Smart including but not limited to store merchandise, technology, cash and store front. Your store key is intended for your personal use to open and close the store or to access the store to perform your duties as a volunteer or employee of Smart Art + Craft Supplies. Your key should always be in your possession and never lent to another person without consent of store management.

I have read this agreement and understand the responsibility of being a key holder.

I agree to immediately report my store key lost or stolen to store management.

I agree to promptly surrender my key at the conclusion of my relationship with Smart Art + Craft Supplies.

I will not in any circumstance make a copy of the store key without prior consent of store management.
