Consistent with HIPAA and HITECH rules, Progress, Inc. has a legal and ethical responsibility to safeguard the privacy of all person(s) supported by maintaining the confidentiality of all protected identifiable information (relating to their support, care, treatment, condition or payment) which is transmitted, maintained or displayed in any form or medium, whether oral, written or electronic.

In the course of my relationship with Progress, Inc. (Smart Art + Craft Supplies), I may hear, see or obtain Confidential Information during the care, service provision or governance of a client.  Such Confidential Information is protected and enforced by federal and state laws in addition to Progress’ policies and procedures.  Such information may be accessed, collected, used, communicated or disclosed only in compliance to the minimum amount necessary to perform duties, if any, for which I am responsible.

Confidential Information includes, but is not limited to, the following information of the patient/client:

Name, address, telephone numbers, age, gender, marital status, insurance information, medical information, psychological information, care or service information, or any other identifying information.

5. SMART BUCKS PROGRAM AGREEMENT

Our Smart Bucks program is designed to recognize and reward our amazing volunteers with store credit to spend at Smart! Smart Bucks are a fun way to say “thank you” for your time, effort, and creativity—but there are a few important guidelines to keep everything fair and consistent:

  1. For You, By You – Smart Bucks are exclusively for use by the volunteer who earned them. They can’t be transferred, shared, or sold to anyone else under any circumstances.

  2. Personal Use Only – Merchandise acquired using Smart Bucks is intended for your own personal crafting or creative projects. It should not be resold as acquired from Smart. Using your supplies from Smart to make art and crafts that you sell is permissible.

  3. No Stacking with Sales – Smart Bucks can’t be used on sale items, discounted merchandise, or combined with any other promotions.

  4. Daily Limit – To help keep things fair and stocked for everyone, Smart Bucks may be redeemed up to $100 per day.

  5. Program Purpose – The Smart Bucks program exists to reward volunteer service and support creativity within our community.

By participating in the Smart Bucks program, you agree to these terms and to use your rewards in the spirit of creativity, fairness, and fun that defines Smart Art + Craft Supplies!
